Privacy Policy
Strados Labs and GoMo Health respect your privacy. Strados Labs Concierge Care® is a software platform with educational content that is available via web links in text messages. This platform is in collaboration with GoMo Health. Neither Strados Labs nor GoMo Health sell, trade, or rent your Personal Health Information, Personal Information, or other information submitted and/or shared while participating in the Strados Labs Concierge Care program to other companies or individuals. Participant information will only be used for its intended purpose.
Strados Labs and GoMo Health may use information collected through the program for research and development purposes. However, such usage will not include any personally identifiable information in such research or development.
Collected Personal Health Information and Personal Information is stored in our secure cloud-based databases, and we reserve the right to divulge information only as mandated to comply with applicable state and federal laws.
At the current time and this point of development, the Strados Labs Concierge Care program is intended for use solely by residents of Pennsylvania while residing within Pennsylvania.
Additionally, the Strados Labs Concierge Care websites are hosted in the U.S. and are subject to U.S. state and federal law. If you are accessing the Strados Labs Concierge Care program from other jurisdictions, please be advised that you are transferring your Personal Health Information and Personal Information to us in the U.S., and by using the Strados Labs Concierge Care program, you consent to that transfer and use of your Personal Health Information and Personal Information in accordance with this Privacy Policy.
You also agree to abide by the applicable laws of any applicable states and U.S. federal law concerning your use of the Strados Labs Concierge Care program and your agreements with us. Any persons accessing the Strados Labs Concierge Care program from any jurisdiction with laws or regulations governing the use of the Internet, including personal data collection, use and disclosure different from those of the jurisdictions mentioned above may only use the Strados Labs Concierge Care program in a manner lawful in their jurisdiction. If your use of the Strados Labs Concierge Care program would be unlawful in your jurisdiction, you are not permitted to use the program.

Participant Information
GoMo Health and Strados Labs aggregate and de-identify the participant information, including Personal Information, voluntarily submitted while using the Strados Labs Concierge program to improve the program performance. GoMo Health and Strados Labs may also receive Personal Health Information about participant health. GoMo Health and Strados Labs may also aggregate the information from these two sources.
Text and E-mail
When a participant sends a text message and/with study-specific data, their mobile phone number and/or e-mail address, the full content of the message, attached files, or other information provided may be recorded. The program may use this information to determine and send appropriate information or educational content to participants. Participants may indicate their preference to stop receiving communications or opt-out at any time. (See your choices regarding your personal information.)

How to Opt-Out:
To opt-out from our SMS service, you can text STOP from your mobile phone to any text that you receive, and we will unsubscribe you from the SMS text messaging program immediately. You will not receive any additional messages unless you re-register.

Security
The program encrypts sensitive information such as your Personal Health Information and Personal Information using secure socket layer (SSL) technology to provide the secure transmission of participant information. In addition, we use commercially reasonable efforts to limit access to your Personally Identifying Information to the employees, agents, and officers who need the information for their jobs. All our employees are kept up to date on our privacy and security practices. Information Collected Via Technology
As you use the Strados Labs Concierge Care program, certain message delivery information may be passively collected and stored by GoMo Health’s, Strados Lab’s, or our service providers’ systems, including your internet protocol address, browser type, and operating system. We also use cookies and navigational data like Uniform Resource Locators (‘URLs’) to gather information regarding the date and time of your website visit and the information viewed. “Cookies” are small pieces of information that a website sends to your computer’s hard drive while you are viewing the site. An internet protocol (or IP) address is a number that is automatically assigned to a participant’s computer when they use the internet. In some cases, an IP address may stay the same from browser session to browser session; but if participants use a consumer internet service provider (ISP), your IP address will likely vary from session to session. The Strados Labs Concierge Care program may use both session cookies (which expire once the web browser closes) and persistent cookies (which remain on the computer until deleted) to provide a more personal and interactive program experience. Persistent cookies can be removed by following the internet browser’s help file directions. Participants may manage cookie preferences via browser settings (see http://www.aboutcookies.org for more information)
Use and Disclosure of Information
In general, Personal Health Information, Personal Information, and other information submitted and/or shared while participating in the Strados Labs Concierge Care program may be used for the following purposes: to enable participant access to program services; to personalize the content received by the participant; to provide additional information and functionality upon request; and/or to improved program services and performance.
Except as otherwise stated in this Privacy Policy, Strados Labs and GoMo Health do not sell, trade, rent, or otherwise share the participant information that we collect with third parties. We may, when required, provide participant information to third-party service providers or affiliates who work on our behalf, to provide some of the services and features necessary to help us communicate with participants. Examples of such services include sending texts or e-mails, analyzing data, processing rewards payments, and providing customer service. Participant information is provided under a Business Associates Agreement (BAA) to affiliates and third-party service providers and is not to be utilized in any manner except as necessary to provide the relevant program services and in compliance with applicable law. If you are not willing to allow the Strados Labs Concierge Care program to use or disclose your collected Personal Health Information, Personal Information, or other participant information, as identified above, you may choose to not participate in the Strados Labs Concierge Care program.
The Strados Labs Concierge Care program uses Google Analytics to analyze how visitors interact with the program website. This service provides non-personally identifiable data including but not limited to data on where visitors came from, what actions they took on the site, and where visitors went when they left the site.
The program may also develop non-personal information concerning visitors to Strados Labs and GoMo Health web pages, using de-identification processes to help ensure that such information cannot reasonably be re-associated or connected with participants. Generally, this
information is de-identified and aggregated to be used in statistical analysis to help analyze patterns of use. This Privacy Policy does not limit the use or disclosure of de-identified information, except as required by any applicable laws.
In the event either Strados Labs or GoMo Health go through a business transition such as a merger, acquisition by another company, or sale of all or a portion of our assets, your Personal Health Information, Personal Information, and other participant information may be among the assets transferred. Program participants acknowledge that such transfers may occur and are permitted by this Privacy Policy, and that any acquirer will continue to process Personal Health Information with HIPAA compliance and Personal Health Information, Personal Information, and other participant information as set forth in this Privacy Policy.
Privacy Policy Changes
Strados Labs and GoMo Health may, in their sole discretion, change this Privacy Policy from time to time. Any and all changes to Strados Labs and GoMo Health Privacy Policy will be reflected on this page and the date new versions are posted will be stated at the top of this Privacy Policy. Unless stated otherwise, our current Privacy Policy applies to all information that we have about you and your account. We may also provide notice to you in other ways, such as through contact information you have provided, and provide you with the ability to opt-in or opt-out of the changes as required by law. Your participation in the Strados Labs Concierge Care program following such changes constitutes acceptance of the revised Privacy Policy. Users should regularly check this page for any changes to its Privacy Policy.
Your Choices Regarding Your Personal Health Information and Personal Information
You may at any time review or change your personal information by going to your account settings (if applicable) or contacting us at support@gold-group.com. Participants have a right at any time to opt-out or to discontinue program contact by replying ‘STOP’ after receiving any of the program text messages. Participants may also contact GoMo Health directly, via email, at support@gold-group.com or contact Strados Labs directly, via email, at support@stradoslabs.com. Upon your request, we will deactivate or delete your account and contact information from our active databases. Such information will be deactivated or deleted as soon as practicable based on your account activity and accordance with any applicable law and legitimate business needs. To make this request, either go to your account settings (if applicable) or contact us as provided above. We may retain in our files some participant information to prevent fraud, to troubleshoot problems, to assist with any investigations, to enforce our Terms of Use, to comply with legal requirements, and to the extent permitted by applicable law.
